Joe has first-hand experience with nearly every aspect of primary care transformation. He has led policy and product development for the AAFP, lead in-the-trenches transformation of a primary care group, and served as national faculty for Medicare’s Comprehensive Primary Care Plus transformation project, and he served as the founding Executive Director of the Direct Primary Care Association. He co-founded Grundy Consulting to work with stakeholders across the industry in order to accelerate the rate of effective transformation in primary care.

Dr. Grundy, known as the Godfather of the PCMH (Patient-Centered Medical Homes), has spent 4 decades focused on population health built on a platform of a healing relationship of trust in primary care. Dr. Grundy is the founding president of the Primary Care Collaborative.

He is a Healthcare Ambassador for the nation of Denmark, One of 7 Honorary Life Members of the American Academy of Family Physicians, Only American awarded an Honorary Lifetime Membership in the Irish National Association of General Practice and the National Association of Primary Care in the United Kingdom.

Evelyn is the back-end glue that holds Grundy Consulting together. Having worked as a non-profit program manager before transitioning to independent graphic design, she is utilizing her skills in program design and implementation, small business management, and graphic design and branding to create, strengthen, guide, empower, and accelerate the business and the transformational work of the team.
